90s: Four Years of Trying Without Pregnancy—Is It Too Late for IVF in the U.S., Mexico, or Japan?

For couples who have been trying to conceive for several years without success, the most stressful part is often not an abnormal test result, but not knowing how much longer they should wait. As more people born in the 1990s reach an important stage in family planning, work, marriage, and family arrangements continue to change, while the time available for fertility planning does not remain unlimited.

 

When Should You Reassess After Trying to Conceive?

 

Under normal circumstances, if pregnancy has not occurred after an extended period of trying, a systematic fertility evaluation should be considered. Women should assess ovulation, AMH, baseline hormone levels, antral follicle count, as well as the condition of the fallopian tubes and uterus. Men should undergo semen analysis to evaluate sperm count, concentration, motility, morphology, and other relevant indicators.

 

If no obvious problem is found after basic testing, but pregnancy still has not occurred for a long time, it should not automatically be assumed that “everything is normal, so we just need to wait.” Age, egg quality, sperm quality, and embryo development can all affect the final outcome.

 

For women, age affects not only ovarian reserve but also egg quality.

 

As age increases, the number of eggs gradually decreases, and the probability of chromosomal abnormalities in resulting embryos may also change. Therefore, women who have been trying to conceive for an extended period need to consider both “how many eggs can still be retrieved” and “what kind of embryos those eggs may ultimately produce.”

 

What Are the Main Steps of IVF?

 

Once an IVF cycle begins, it generally involves ovarian stimulation, follicle monitoring, egg retrieval, fertilization, and embryo culture.

 

Ovarian stimulation is not simply about pursuing the highest possible number of eggs. Medication is used to encourage multiple follicles to develop simultaneously, while ultrasound and hormone levels are monitored so the treatment plan can be adjusted according to the individual response. After egg retrieval, the eggs are fertilized with sperm, and the resulting embryos continue to develop in the laboratory.

 

Some embryos can develop into the blastocyst stage. For families who have already spent years trying to conceive and want to understand their embryos more systematically, IVF-PGT, also known as third-generation IVF embryo genetic testing, can be performed at the blastocyst stage. This analyzes the chromosomes of embryos and provides additional information for subsequent embryo selection and transfer.

 

“After trying for so long, is it already too late to consider IVF?” This is a common question when people begin seriously considering assisted reproductive treatment.

 

The answer cannot be determined by the length of time spent trying to conceive alone. Age, ovarian reserve, sperm condition, and previous test results all need to be considered. Four years of trying does not necessarily mean that the opportunity to have a child has been lost, but it does mean that continuing to wait without a clear direction may not be the most appropriate approach.
